From Patient-Centered Care to Person-Centered Health: The Effective Solution for Delivering Quality

For several years, there has been a focus on patient-centered care, also referred to as person-centered care, which encompasses: respect for people's values, preferences, and expressed needs; coordination and integration of care; information, communication, education; physical comfort; emotional support and alleviation of fear and anxiety; involvement of family and friends; and transition of continuity.*

 

However, with a move towards a more integrated approach to health care, there is a need for health care organizations to not only manage and deliver care, but to also manage and deliver heatlh in partnership with the person and their family. Doing so will lead to a greater emphasis on person-centered health.

 

This is not a debate about words, but a journey towards a truly integrated person-centric model, whereby the accountabilities and needs of the individual and the provider's offerings are understood, considered, and included in the health process.

 

This conference will encourage discussion and debate on the components of a person-centered health model.
