Chronic Disease Management: System Solutions for Sustainability

Chronic disease is widely recognized as the major cause of world-wide
death and disability. In Ontario, a large and growing number of people
live with one and commonly two or more chronic conditions. Poorly
managed or untreated conditions pre-dispose individuals to additional
chronic illness. The health and economic burden of chronic disease is
high—to individuals, health systems and the economy.
The OHA in partnership with the Ontario Association of Community Care
Access Centres, the Ministry of Health and Long-Term Care, Cancer Care
Ontario and the Ontario College of Family Physicians is hosting this
conference which will offer participants practical solutions to the
chronic disease challenge for today and tomorrow through a collaborative
systemic approach which is applicable both locally and provincially.
